Housing Case Manager

JoB Description

The Housing Case Manager provides a variety of office and field activities to families who are, or are at-risk of, experiencing homelessness to promote housing stability. Primary activities involve outreach, familiarizing eligible participants to programs, providing housing search and strength-based, housing-focused case management services, conducting assessments, evaluations and referrals, while maintaining accurate and timely records.

This is a Regular Full-Time/Non-Exempt that reports to the Director of Operations.

 


ESSENTIAL DUTIES AND RESPONSIBILITIES:
The essential functions include but are not limited to:

  • Housing Case Management
    • Assess housing barriers & assist participants in development of a strength-based/solution-focused individualized goal and action plan that promotes permanent housing and self-sufficiency; develop an effective, timely referral network of social service and community programs to ensure ongoing direction and support as needed;
    • Assist in development of and encourage adherence to a personal budget through pro-active housing and budget counseling sessions; provide support in establishing payment plans for bills and past debt to assist participants in obtaining and maintaining housing;
    • Survey rental market for affordable housing and develop housing lists for participants; assist participant in locating and securing housing of their choice; assist with housing applications; advocate with prospective landlords;
    • Coordinate move to permanent housing; and provide mediation and advocacy with landlords on participant’s behalf to develop a workable plan to obtain or maintain housing;
    • Apply knowledge of residential lease contracts to educate participants of their rights and responsibilities;
    • Provide pro-active follow-up home visits to ensure stability and further progress towards self-sufficiency; this includes support, advocacy, reducing isolation, listening, problem-solving and identification of social supports to assist with establishing community network;
    • Create and maintain consistent communication channels between several parties (i.e. tenant, landlord, collaborating partners, debtors, and creditors);
    • Determine eligibility for prevention/diversion funds and recommend amount; allocate and track use of funds;
    • Adhere to strict boundaries and professional ethics in the care of others;
  • Maintain Client Records
    • Maintain accurate and timely documentation of services and outcomes in accordance with Federal, State, County and agency guidelines;
    • Complete a Service Prioritization Decision Assistance Tool (SPDAT) for all families within two weeks of program referral;
    • Complete all required and ongoing training for the Chester County Client Information Management System and input participant case notes and records within 72 hours of activity;
  • General
    • Attend professional development and learning opportunities through training, workshops, and conferences;
    • Submits program summary reports and data to Senior Management on a monthly basis
    • Attends and participates in all staff meetings, monitoring sessions, community events, and other agency related business.
    • Perform other duties as assigned to effectively conduct the Agency’s business

 



QUALIFICATIONS, KNOWLEDGE, SKILLS REQUIRED:

  • Bachelor’s degree in an applicable human services field plus two years of professional social work/case management experience with homeless and/or at-risk populations.
  • Knowledge and belief in “Housing First” philosophy and strategies and strength-based case management.
  • Demonstrated knowledge of community resources, social service agencies, and landlords.
  • Excellent communication skills, particularly listening, mediation, and writing skills/
  • Strong organizational skills with ability to meet a demanding workload.
  • Ability to work collaboratively with other personnel and/or service providers or professionals.
  • Proficiency in Microsoft Office and related products.
  • Experience working with a diverse population, with sensitivity to differences in culture, religion, sexual orientation, race, age, and physical and mental illness and commitment to treat each individual with respect, dignity, and compassion.
  • Candidate must have a valid driver’s license and a car. And will be required to pass a criminal background check and child abuse clearance.
  • Spanish fluency a plus.



 

REQUIREMENTS:
Prior to date of Hire:

  • complete FACPC application, providing three references;
  • submit current PA criminal and child abuse clearances;
  • pass DMV clearances.
